Our aim is to help identify your priorities and challenges, and help make recommendations and connections for you with the most appropriate resources so that you can be confident and prepared that you have support, whatever your future holds.  From Proactive Aging Plans evaluating your preparedness for your aging journey. Elder care and transitions involve the overlap of multiple spheres.  They encompass the medical, financial and legal systems.  Navigating and communicating within and between these systems can be frustrating.  This adds to the burden of individuals and families in transition.  We are here to assist you to navigate across all of these systems.  Either working with your existing network of trusted professionals or we would be happy to introduce you to professionals within our own trusted network.  It is our aim not only to relieve tensions in a current crisis or situation, but to assist you in planning to minimize disruptions as your needs change.

We will walk with you every step of the way, facilitating difficult conversations with family.  Addressing your needs and your goals, and taking on some of the multiple steps required to access what you need to age on your terms.
